        CLSI  ADMISSION GUIDELINES AND POLICIES        

  1. The student's registration in CLSI is considered an expression of the student-applicant and his/her parent's/guardian's willingness to abide by all its rules (Ref.: The CLSI Student Handbook).

  2. CLSI reserves the right to refuse acceptance ( re-admission) of any student who has undergone our standard assessment.

  3. Only students with satisfactory academic qualifications,  of good moral standing, and with a socially-acceptable personality will be considered candidates for re admission. Newly accepted students shall  be considered under probation or observation, pending unforeseen disruptive behavior that may affect the conducive condition, thus,  jeopardize the welfare of each student.

  4. Only student-applicants with COMPLETE pertinent DOCUMENTS will be entertained.

  5. Students will be admitted on a “FIRST-COME-FIRST-SERVE” basis.

  6. Voluntary disclosure of applicant's difficulties (personality, behavioral, psychological, etc.). with or    without specialists' diagnosis; student/s with special needs shall comply/submit the following:

    • Official/Clinical diagnosis of a Developmental  Pediatrician must be submitted;​

    • Pertinent current/latest intervention /therapist report/s 

    • The Parent's/Guardian's willingness to voluntarily commit to a Memorandum of Agreement with CLSI  for the student's entire stay in school, if he/she will be accepted;

  7. CLSI reserves the right  to deny admission/ readmission to an applicant/student with special needs based on capacity to manage.

  8. CLSI reserves the right to accept or refuse acceptance of FOREIGN students who intend to stay on a temporary basis; Foreign students shall be willing to pay the  tuition and miscellaneous fees IN FULL, covering  the entire current academic school year.  The stipulated school guidelines and policies shall apply to both local and foreign students duly enrolled in CLSI.

AGREEMENT

  1. To pay without demand all tuition fees, other charges and fees, in accordance with the schedule of payments. Failure to do so within one month from due date gives CLSI the right to terminate the child's enrolment so that his/her space be made available for another.

  2. That CLSI, its officers, teaching staff, and employees are not liable pecuniarily or otherwise, for any accident and/or injury suffered by the enrollee in its premises, particularly when such accident and/or injury is caused by the the act of a fellow enrollee, fortuitous events or by persons not connected or employed by CLSI.

  3. In the event enrollee suffers any injury for any reason, consent is hereby given that enrollee may be brought by the school to any physician or hospital for emergency treatment, with costs of treatment, medicine and related expenses for my/our account (over and above the coverage of medical insurance).

  4. That I/we understood and shall comply to  the CLSI Admission guidelines, policies and procedure presented to me/us.

  5. That I/we agree to have my/our child's picture/video posted in any publication by the school.

  6. That I/we hereby certify that all information  in this application is complete and accurate, I/we understand that declaration of incomplete or inaccurate information may be ground for non-acceptance to CLSI.

 

NOTE:   For re-enrolment purposes, this Student Profile Form  shall serve as official reference unless  the School is notified to have it replaced by the Parent/Guardian.​
