2. Interview with CLSI Guidance Counselor

3. Assessment of Fees and Payments

 

a) ​Reservation Fee of P5,000.00 (non-refundable but deductible from the Tuition Fee)    

b) After an assessment is made, kindly proceed to online payment

   4. Initial Documents Required

a) Report Card/Form 138 (Kinder-Grade 10 only)
b) Photo copy of PSA Birth Certificate
   

Other requirements:
a)    2 colored photos (1X1 size) for ID card
b)    CLSI Recommendation Form (Grades 1-10 only)
c)    Transcript of Record (SF 10) to be sent by previous school (Grades 1-10 only)
